EALING will receive £1,417,568 as part of a programme to help councils ease winter pressures on the NHS.
The aim is to get patients home quicker and free up hospital beds.
The extra funding is aimed at reducing delayed transfers of care and could pay for the following:
- home care packages to help patients get out of hospital quicker
- re-ablement packages to help patients carry out everyday tasks and regain mobility and confidence
- home adaptations, such as adapting a shower room if a patient has limited movement
London councils will receive £37,170,352 overall.
The boroughs have been allocated funds based on a relative needs formula.
